We are in network for Blue Cross Blue Shield PPO, Tricare, and Medicare. We also accept out of network insurance. For your convenience, we will contact your insurance, to get a quote of benefits. However, we suggest that you also receive confirmation of your benefit package.
We also accept self-pay.
We are located at 2272 W. 95th Street Suite 305 Naperville, IL 60564. We are next door to Neuqua Valley High School in between Route 59 and Book on 95th Street in a red brick three story building.
Please contact your therapist to cancel or reschedule your appointment. We have a 24 hour cancellation policy. If 24 hours is not given, a $75 will be charged to the individual, not the insurance company.
